Monthly Paperwork

Please complete the Volunteer Diary after each family visit and send to the office at the end of each month along with your monthly expense form for your co-ordinator to authorise payment.

If you have receipts for travel or parking please ensure these accompany your claim as payment cannot be made for these items without.  Your expenses need to be given/emailed to your co-ordinator who will authorise these prior to the monthly payment run; in order for your expenses to be included in the payment run we need to have received them no later than the 5th of the month.  You will find all your paperwork in the orange box at the top right of this page.

The expense form has formulas inputted into the shaded areas which will automatically calculate your expenses for you.

IMPORTANT - Please remember to sign your expenses and input the number of miles for each round trip to avoid delays in payment as your expense form will be returned to you. 

If you would like to donate your expenses back to the scheme to be used towards family events please indicate this on your completed form.
